Health Plus Ortho Management, LLC, Orthopaedic Specialty Surgery Center, LLC and associated Physician Owners (the “Applicants”) are filing a Certificate of Need application pursuant to Section 19a-638 of the Connecticut General Statutes for the transfer of ownership of an outpatient surgical facility located at 40 Old Ridgebury Road, Suite 201, Danbury, CT 06811 (the “ASC”). The Applicants will request CON approval for Health Plus Ortho Management, LLC, which already owns 40% of the membership interests in the ASC, to acquire an additional 17.7% of the membership interests in the ASC. The capital expenditure in connection with this acquisition will be approximately $7,257,000.
